The Case!
Setting
A patient-centered medical home that is part of a safety net hospital network. Patients at this clinic have many socioeconomic factors that limit access to care.
Patient Demographics
PW is a 76-year-old Hispanic male.

Reason for Visit
PW was referred by PCP to the ambulatory care pharmacist for dyslipidemia management.
One possible consideration would be to do nothing since no statin (any lipid lowering therapy) has been prescribed/taken for 10 years. How can you make him better by giving a drug now. Certainly, would not consider high intensity therapy at this point. SHARED DECISION MAKING should include at 76 years old, the evidence is weak to show you will be better off.
